The "Run-time Error 5" problem is that the menu item or icon that starts up Quality Analyst has the location for Quality Analyst specified using the "UNC" server name convention (the "\\..." form). Quality Analyst needs to be referenced from a mapped drive letter.
This is set in the Windows Properties for the menu item or icon or shortcut that you use to start up Quality Analyst. Go to the Properties for the menu item, and select the Shortcut tab. Change the drive specified in both the "Target" and the "Start in" to use a mapped drive letter (and directory name as needed), not the "\\" server name notation.
