I would like to enter data in such a way that when the operators find a fault on a board, they not only record the fact that they found a fault but they also record the fault type & location of the fault. The aim being that not only can they see how many of a certain type of fault occurs but also see if it always occurs in the same location.

The only way to do this is to have one record per fault and location. ie the sample size is one.
